In accordance with the General Data Protection Regulation of April 14, 2016, hereinafter referred to as GDPR, we advise that:

1. The administrator of your personal data is HI Orthotics based in Warsaw at Aleje Jerozolimskie 89/43.
2. HI Orthotics will process your name and surname, e-mail address, and contact details of your orthotist.
3. Your personal data will be processed for the purpose of responding to your inquiry and direct marketing.
4. The legal basis for the processing of your personal data is your consent to their processing.
5. Providing your personal data is voluntary.
6. Your personal data will be processed on behalf of the data administrator by authorized employees only for the purposes referred to in section 3.
7. Your personal data will be stored for a period of 3 years.
8. Your personal data will not be made available to any third parties, except for cases provided for by law.
9. Pursuant to the provisions of the GDPR, you are entitled to:
   • the right to access your data,
   • the right to rectify them if they are incorrect,
   • the right to delete them, limit processing, and transfer data – in cases provided for by law,
   • the right to object to data processing,
   • the right to lodge a complaint with the supervisory authority, if you believe that the processing of your personal data violates the provisions on personal data protection,
   • withdrawal of consent at any time without affecting the lawfulness of processing based on consent before its withdrawal.
